Office Hours
Our office is open Monday – Friday for your convenience. Normal business hours are 8:00 am until 5:00 pm Monday thru Thursday, and 8:00 am until 12:00 pm on Fridays.
Appointment Scheduling
Most visits can be accommodated with your schedule in 1-2 weeks at our office. We also welcome ‘emergency visits’ or same day problems.
If you are a new patient to our practice, please plan to arrive at least 15 minutes before your scheduled appointment time. Be sure to print out and complete your paperwork to bring with you to the visit.
Cancellation Policy
We require a 24 hour notice for all cancelled or rescheduled appointments. A charge may be applied to your account if appropriate notice is not given.
Insurance Information
Our office accepts most insurance plans, including EyeMed vision plans. Below is a list of the most common plans we participate with. If you do not see your plan listed, please call your insurance carrier to find out if we are on their panel.

Payment Information
For your convenience our office accepts cash, personal check and credit card payments by Visa, Mastercard and Discover. We require payment due as services are rendered. If you are being seen in an emergency situation and need to make payment arrangements, those will need to be approved prior to receiving services.
NSF Checks: Any check that does not clear your bank account will result in a $30.00 fee.
Billing Policies
As a courtesy to our patients we will gladly bill your medical or vision claims for you. While the filing of your insurance claims is a courtesy we extend to our patients, any unpaid charges are patient responsibility.
Any out of pocket expenses, such as deductible, coinsurance and/or copays are due at the time of service. If you do not bring your copay with you to the appointment, we will gladly reschedule the visit.
If you do not have insurance, payment in full is due at the time of service.
If there is a balance on your account after filing your insurance carrier, you will receive a statement. Payment is expected within 30 days of receiving your statement. Accounts with unpaid balances after 30 days will be considered delinquent, and may be imposed an additional fee by collections process.
Emergency Instructions
Normal office hours are Monday – Thursday 8:00 am until 5:00 pm and often closing at 12:00 pm on Fridays. If you are experiencing a life threatening emergency, please go to your nearest emergency room.
If you are experiencing an ocular/eye emergency, after hours, please call 859-276-2594 to reach the ophthalmologist on call.
